One thing that can really take a toll recovery plans, is when patients spend extra days in hospital waiting for discharge. Muscles don't get used, so they weaken and atrophy. This makes recovery at home much harder and longer.

People in the UK are less likely to survive treatable conditions, such as breast cancer and stroke, than those in other rich nations, a study has found. By the time people actually seek care for their conditions, the care is mostly limited to bathing, feeding and medication, and decline sets in with limited chances of health recovery.

A study published in Health Watch also reported that there were 484,609 emergency readmissions to hospital within 30 days, with with one in five cases readmitted within 24 h of discharge (‘failed discharge’) from hospital. According to recent data on 30 day hospital readmissions linked to lack of home care and support. Around 14% of patients in England are readmitted within 30 days of discharge, with rates rising to 18% among those aged 75 and over. Many of these readmissions are directly tied to inadequate post discharge care, poor medication management, and insufficient support at home

Why Patients Sometimes Go Back to Hospital

In fact, around 1 in 7 patients in England are readmitted within 30 days of going home, often because they don’t get the right care or support after discharge. The three main elements that cause hospital readmissions is a lack of co-ordination, lack of a discharge plan, and the failure of NHS services to cater to complex patient needs. Here are the main reasons why readmission happens:

1. Care That Doesn’t Connect

When different doctors, nurses, and services don’t share information properly, patients can feel lost. Missed appointments, unclear instructions, or poor communication between hospital, GP and community care teams often mean that patients, wait for care, or receive only the most basic care and end up back in hospital.

2. Weak Discharge Planning

Going home without a clear care plan is risky. Patients need:

  • Easy to follow instructions about medicines

  • Follow up appointments already booked

  • Support at home, whether from family, carers, or community services

Without these, recovery can stall, and problems quickly build up.

3. Complex Needs

Some patients face bigger challenges. Older age, long term health conditions, mental health struggles, or living alone without proper care or support, makes recovery harder. They will more likely to need extra help to stay well at home.

If you or a loved one is a patient in hospital and have received a discharge assessment or order that has determined the patient is ready for discharge, but it's not released, because the discharge is not organised, or the patient needs more care after leaving hospital, and the hospital has delayed discharge.

This essential advocacy service package helps ensure you have a timely discharge by making sure you have a carer (if necessary) in place, transport home from hospital arranged, a plan for preparing your home ready for your return to be picked up. make sure there is food and drink in the house and, if it is cold, put on the heating, Make sure you have house keys with you, Make sure you have suitable outdoor clothing to wear, all your belongings and medication when you leave hospital.
